Project Objective

To develop and evaluate the effectiveness of the “dignity@home™” mobile application, which incorporates a tailored referral algorithm for home-based palliative and hospice care, aiming to facilitate timely utilization of such services for end-of-life patients receiving home nursing care.

 

[Phase 1 Objectives]

– Select the most suitable existing palliative and hospice referral tool for application in home nursing settings.

– Develop a personalized referral algorithm for home nursing by:

Extracting symptoms, treatment preferences, and temporal characteristics from counseling records of end-of-life patients receiving home nursing care through natural language processing.

Utilizing electronic health records to obtain personal, social, environmental, and clinical information.

Applying machine learning techniques to integrate the above data into a customized referral algorithm.

 

[Phase 2 Objectives]

            – Develop the “dignity@home™” mobile application incorporating the referral algorithm.

            – Evaluate the application’s effectiveness through both qualitative and quantitative assessments by end-users.
